CLE Sample Forms
State of New York
- Sign in on the CLE sign-in sheet (Use the CLE Course Attendance Form for Annual Meeting)
- Return your completed evaluation form to the Registration Desk.
- Sign the New York Attendance Sheet (State reuirements are for attendees to sign in and out)
- Pick-up the New Yok Certificate of Attendance sheets.
- Obtain ABA staff signature on New Yok Certificate of Attendance sheet (this is a mandatory starte requirement).
- Return signed pink/yellow copies to ABA Staff. Retain the white copy for your records.
- The ABA Center for CLE will submit the sign-in sheets and signed certificates to your state bar.
